H-1B Temporary Worker Visa Checklist & Information Sheet
In order to properly document your petition, it is necessary for you to obtain the following items & complete this H1B information sheet:
I. Documentation regarding the Beneficiary/Employee
• Current Résumé.
• License or other authorization to practice profession if applicable.
• Copies of all post-secondary degrees and diplomas.
• Copy of all transcripts for all college degrees, post-secondary degrees
• If education completed outside of the United States, academic credential evaluation (if you do not already have one, we can arrange to obtain one for you).
f.. If you have previously visited the United States or held any visas, please include copies of them.
• Copy of all of the pages of your passport with writing or stamps.
• A copy of the front and back of your I-94.
• If you will also petition for a spouse or children, provide a copy of their passports, front and
back of their I-94 forms if in the U.S., a certified copy of the Marriage Certificate (if not in English, with a certified translation), and copies of Children's Birth Certificates (if not in English, with a certified translation).
